If you think you may require any vaccinations relating to foreign travel you need to complete a travel risk assessment form and hand in to reception at least 6 weeks prior to departure. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

It is important to hand this form in as early as possible – at least 6 weeks before you travel – as 2 appointments will be required with the practice nurse, a telephone consultation and then a face to face appointment to actually receive the vaccinations and be given the relevant health advice. These vaccines have to be ordered as they are not a stock vaccine. Your face to face appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

Minor Surgery
Doctor performs minor surgical operations within the surgery premises. This means that you may not need to go to hospital for small operations e.g. injection of piles, removal of cysts and warts and joint injections.

Pre-pregnancy advice, including infertile couple counselling is available.

Ante Natal & Post Natal Clinics – Midwife with the Doctors
Ante natal clinics are held at a local practice, booked by your midwife. The practice has a midwife attached who practices from our surgery
Post natal appointments are held at the practice by the doctors. This appointment should usually take place 6-8 weeks after the birth of a baby, and checks both mother and baby. It is important your baby attends their Post Natal check in the time-frame suggested.
Children’s Clinic – Health Visitor led at various locations

Health Promotion (e.g. Dietary advice), Chronic Disease Management, Cardiac Disease Prevention, ECGs, Diabetic screening, Asthma checks, Blood Pressure checks, venepuncture, Immunization for children, adults and travellers, Cervical smears, Minor surgery, Breast examination advice, Dressings, removal of sutures, ear syringing, contraception, over 75’s health checks, specialist drug monitoring e.g. Warfarin, Methotrexate, etc.

Health and Well-being Coach
The Health and Wellbeing Service aims to help patients in the Nottingham West Primary Care Network take more of an active role in their own health care through structured Health Coaching consultations. There is a focus on changing detrimental lifestyle behaviours with the intent of reducing health risks, improving self- management and symptoms of chronic conditions.
The Health and Well-being Coach can guide and support patients to set realistic and achievable health related goals to improve quality of life and overall health and wellbeing. The criteria for the service includes all patients living with Diabetes or pre-diabetes and all patients living with a respiratory condition. The Health Coach offers 1:1 support as well as group exercise sessions and support sessions. Sessions can be conducted virtually, face to face or over the phone according to the patients preference.

The Nottingham West Digital Inclusion Team are part of a new project aiming to help people in the local community to better manage their digital healthcare using the NHS app and Patients Know Best (PKB). The NHS app and PKB are digital platforms that enable you to find health advice, orderprescriptions, manageappointments, access your COVID pass and much more. They allow you to take control of your healthcare, access services at a time that suits you, and reduces the need for unnecessary trips to the doctor’s surgery (see below for more features & benefits).
The Digital Inclusion team, comprised of Damian, Paul, and Pauline, understands that navigating digital platforms can sometimes be daunting, and they are committed to providing 1-2-1 support to make things easier, and help you overcome some of technical barriers you may face. They can guide you through the process of downloading and setting up the NHS app on your device*, verifying your identity, and technical help with issues such as resetting your password. They will also provide you with general advice on how to use the key features of the NHS app in ways that best suit you!
